Municipal offices: PESEL, address and local services

Find the correct municipal department and keep evidence of your application or request.

Choose the service and office

A municipal office, urząd gminy or urząd miasta, handles services such as PESEL and address registration. Local family or social assistance may instead be handled by a designated OPS, MOPS or CUS. Identify the service on the official municipal website and check its jurisdiction: some services depend on your address, while an ordinary PESEL application can be made at any municipal office.

Prepare your documents

Check the current form, identity documents, evidence and any representation requirements for the chosen service. If an authority requires you to obtain PESEL, ask it for the legal basis to enter in the application. Ordinary PESEL assignment is free. Other services may have different fees; pay only to the official account specified for that procedure.

Submit and keep confirmation

Use an accepted channel and sign the form as required. Check whether personal attendance, an appointment or documents for a child are needed. Ask for a receipt or reference number and retain a copy of the application and attachments. A telephone conversation or appointment reservation alone does not establish that a formal application was filed.

Check the result and next steps

Respond to requests for missing information and correct inaccurate records. PESEL, active UKR, address registration and residence permission are different matters. If refused, ask what formal document is issued and follow its remedy instructions. A general service complaint does not replace an appeal or suspend its deadline. Use the separate Documents guides for detailed PESEL and meldunek steps.
